[U-Boot] [PATCH 2/3] USB: Rework usb_kbd.c

* Support dynamic allocation of devices
* Passing data via usb device privptr
* Reorder functions to avoid forward declarations
* Introduce generic polling mechanism to fix musb and ehci-hcd breakage
  due to using "extern new;" to access keyboard driver data!
