There are a couple of formal issues here:

Please change "Reviewed by:" into "Reviewed-by:" and "Tested by:" into
"Tested-by:" - no, this is not nitpicking, some automated tools rely
on the correct spelling.

Also, all these entries require a real name, so please change
"prafulla_wadaskar" into "Prafulla Wadaskar".
