Fixes:
        o       fixed long dir entry slot id calculation in get_vfatname
 
Changes:
        *       fixed long dir entry slot id calculation in get_vfatname
        *       replaced all 0x40 in fs/fat/fat.c with
                LAST_LONG_ENTRY_MASK and added LAST_LONG_ENTRY_MASK to
                include/fat.h
