Cover letter subject indicates [PATCH 0/3] as you're sending cover 
letter plus 3 patches but actually you're sending 4.

You should retry to:
# git format-patch -s -M -o outgoing -4 --cover-letter

and then:
# cd outgoing
edit cover-letter(patch-0000)
# cd ..
# ./scripts/get_maintainers.pl outgoing/*
# git send-mail --to "..." --cc "u-boot..." outgoing/*.patch

This way works, obviously you have to substitute "Names <emails>" on 
--to --cc
